Family Nurse Practitioner, Acute Care, Primary Care specialty.

Job purpose

Employee is expected to work independently under the collaborative agreement to plan and perform complex individualized treatment plans.

Duties and responsibilities

Eligibility to carry 30 (100 when eligible) per month caseload in MAT. See 12 -15 established Primary Care patients as assigned:

Determine the medical eligibility of potential clients seeking admission into the facility and coordinate the pre-admission process with Nursing and Admission staff.

Approve admissions to detox and residential treatment. Initiate Standing Orders.

Perform a history and physical examination of each admitted patient within the approved time frame.

Coordinate post-discharge referrals for ongoing medical care, as appropriate. Provide a discharge summary and discharge instructions for each patient according to

protocol.

Provide leadership to nursing staff in medical/behavioral situations as needed.

Taking “call”, being available for phone consultation during non-clinic hours and weekends. In intermittent coverage for the Vice President of Medical Services

Perform duties within the context of the latest Joint Commission standard policies and plans.

Regularly meet with Vice President of Medical Services to review progress of the organization’s programs.

Assist in the efficient operation of the medical function providing input on billing and equipment.

Responsible use of confidential data understanding that indiscretion can have serious consequences to the patient and the agency. Tactful interactions with colleagues in and

outside the agency encouraging cooperation.

Participate in weekly MAT staff meeting when working in MAT clinic.

Participate in scheduled Primary Care staff meetings.

Participate in efforts to ensure that the organization meets and surpasses Joint Commission, managed care and other accreditation standards relative to medical services.

Assist with and participate in NIATx improvement processes as assigned.

Participate in peer review as assigned.

Qualifications

A Nurse Practitioner with an unrestricted license in the State of Ohio and Board Certified to practice Family Nurse Practitioner scope. Preferably at least 1 year of experience in

Family Practice. The Nurse Practitioner is required to have a current waiver to prescribe buprenorphine to 30 clients per month, with eligibility to advance to 100 clients per

month. Medicaid provider number in Ohio is required, as well as eligibility and willingness to obtain a Kentucky Medicaid provider number.

Physical requirements

• While performing the duties of this job, the employee may need to remain in a

stationary position for long periods of time or may need to move about for extended periods of time.

• Position may require employee to have full range of motion and lift up to 30 lbs;

• Position may require individual to communicate, converse and exchange information with others.

• Position may require individual to perceive communications via email, phone or in person.
