Description
Position Title: Temporary Asthma Navigator
Division: Pulmonary
Duration: 1 year
Salary range: $18.59 - $22.37
Representative Responsibilities:
- Participates as member of the multi-disciplinary team and assists with the coordination of care among children with asthma, their families, their school health teams, their health care providers, and community resources including face to face home visits to both children with asthma and their families, providing asthma education, assessing asthma control, and identifying social determinants of health (SDOH) needs and gaps in care and asthma management.
- Communicate with families and the care team through multiple channels such as in-person visits, community/school events (prioritized), phone calls, emails, faxes, letters.
- Assess and help address social determinants of health, including referrals to multi-disciplinary teams and community resources/organizations.
- Facilitate communication with care providers across hospital and community-based care settings concerning asthma management including parental concerns, SDOH needs and care gaps
- Partner and collaborate with community partners such as schools (including school health teams) to identify students with asthma and provide support for students with asthma, such as assessing asthma control, and providing tailored education and support
- Collaborate with school health teams to understand the community and school environment and build a supportive infrastructure for collecting information related to asthma.
- Organize, coordinate, and deliver asthma-related interventions to children/families in coordination with their asthma providers, primary care providers and school health teams
- Collect and submit data including process-related metrics in a standardized manner to the ALHS team on a scheduled basis. Team meetings will be primarily conducted via teleconferencing. Summary reports will be presented to the ALHS team on a quarterly basis
- Attend trainings and ongoing professional development opportunities to further competency in asthma management, community health navigation, and specific developed interventions.
Qualifications
EDUCATION/EXPERIENCE
Minimum Qualifications:
- Experience with Microsoft Outlook, Word, Excel, PowerPoint
- Experience with conducting Virtual Meeting Sessions (i.e., Zoom, Microsoft Teams)
- Ability to meet deadlines and adapt to the changing needs of the patients
- Access to a reliable internet connection in a reliable quiet location
- Ability to work variable hours, including evenings and weekends depending, and travel independently throughout the assigned region using own method of transportation
- Organizational skills with ability to prioritize multiple tasks and meet deadlines
- High school diploma or equivalent
- Current Driver's license
Preferred Qualifications:
- Current Community Health Worker (CHW) certification in the state of Ohio
- Demonstrated commitment to inclusiveness, health equity, and reduction of health disparities.
- Experience in working with community-based health programs, clinical and community health service providers.
