Description

Position Title: Registered Nurse II, Non-Acute

Department/Unit: Human Genetics

Shift/Hours: Days/ 32 hours/week 4 eight-hour days

Expected Salary Range: Minimum $34.50 - 43.54; Mid-point; Salary determined based on years of nursing experience.

Combination of in clinic patient facing and office setting. Seeking a nurse to coordinate care for patient populations of metabolic conditions: Abnormal newborn screens, PKU conditions, establishing multidisciplinary care, coordination with outside pediatrician, institutions.

Metabolic assessment, problem identification and triage, problem resolution such as scheduling appts, education reinforcement, coaching/counseling, assisting with med refills, supply/metabolic food products, documentation in EPIC, nuances between state Medicaid and product availability, ordering, authorizing.

situational assessment, record review and clinical preparation (pre-visit planning), assisting to obtain and maintain a new therapy/medication/formula, adjustments made for growth, management of patients off registry, long term plan of care development, care coordination and goal management, school forms, order management, management of peri-op, acute illness, emergency letters.

Follows professional practice standards and guidelines. Qualifications

EDUCATION/EXPERIENCE

Required:
·Associate/Diploma prepared RNs from ACEN, CCNE or NLN CNEA accredited inst with 2 yr. exp.
·If hired as an external candidate without BSN or MSN, must enroll in a BSN or MSN program within 1 year of hire date and complete program within 5 years of hire date
· Active Ohio RN License. May be required to obtain other state licensure.
· 2 years of work experience in a related job discipline.
·Foreign graduates must have license validated through CGFNS per Ohio Board of Nursing code for Foreign Graduates

Preferred:
·BSN from ACEN, CCNE or NLN CNEA accredited institution or MSN
