Status: Full-time (37.5 hours/week)
Pay Range: $45,000.00 - $60,605.00
Work Location: Community- The Assessor must be able to drive to and from client/member homes and other community settings in the COA 5-county area, work from own home and travel to office for training and meetings.
Job Summary:
The Assessor is responsible for conducting person-centered assessments to determine eligibility for various programs and services. This role involves providing education and support to individuals and their caregivers and ensuring that all documentation and compliance standards are met. The Assessor will work collaboratively with internal staff and external partners to advocate for clients and connect them with appropriate resources.
Essential Functions:
- Complete face to face assessments in the community with Consumer and caregiver/contact as appropriate to determine level of care per ODA and COA policy.
- Document in appropriate software system
- Communicate with waiver service facilitator as appropriate
- Communicate with Managed Care Organization per policy
- Work cooperatively and constructively with internal COA staff, and with formal and informal systems to communicate consumer needs.
- Advocate on client’s behalf particularly if there is a health and safety issue or if client is at risk for institutionalization.
- Ability to complete assessments during core business hours, early evening or weekend hours as needed to meet the needs of the community.
- Complete incident reports as indicated.
- Follow firewall requirements of confidentiality and neutrality as it relates to the MyCare Ohio Program.
Required Education, Licensing & Certifications:
Ohio licensed Social Worker, Licensed Practical Nurse (LPN), or Registered Nurse in good standing with the credentialing board.
Experience:
At least one-year prior experience in medical social work, home health care and/or geriatrics.
Skills & Abilities:
- A good working knowledge and understanding of issues related to the older adult population.
- Knowledge of the individual, family, social systems and the individuals functioning in the aging process.
- Prior experience completing electronic documentation a plus.
- A good working knowledge and understanding of issues related to the elderly population.
- Critical thinking skills and the ability to effectively use clinical judgment.
- Ability to learn in-person, through a written format, and virtually.
- Tolerant, empathetic, good interviewing capability.
- Knowledge of community resources within Southwestern Ohio.
- Good customer service skills.
- Ability to adapt to frequent change.
- Good oral and written communication skills.
- Good organizational and teamwork skills, including the ability to collaborate and solve problems in a self-directed team environment.
- Proficient computer skills including the Windows operating systems, MS Word, MS Excel, data entry and the ability to learn new software applications as needed.
- Respect the cultural, spiritual, racial, ethnic beliefs of others.
- High speed internet in own home used for business purposes of accessing agency systems.
