Job Title: Care Manager (LSW)
Location: Field based - Southwestern, Ohio (Hamilton, Clinton, Clermont, Butler, and Warren Counties)
Job Type: Full-Time (37.5 hours/week)
Pay Range: $49,000 – $66,000 per year (commensurate with experience)

What You’ll Do

• Conduct in-home assessments to determine eligibility and service needs
• Develop and monitor individualized care plans
• Coordinate services such as personal care, meals, and transportation
• Advocate for clients and connect them with community resources
• Maintain accurate documentation and ensure compliance
• Collaborate with interdisciplinary teams and attend ongoing training

What You’ll Bring

• Licensed Social Worker (LSW) in Ohio (required)
• 1+ year of experience in case management, social services, or healthcare
• Strong communication and organizational skills
• Ability to manage a caseload independently
• Valid driver’s license and reliable transportation
• Proficiency in Microsoft Office and electronic documentation systems
