Job Title: Care Coordinator
Location: Positions available in Clinton and Hamilton Counties (Field-based home visits with remote flexibility)
Job Type: Full-Time (37.5 hours/week)
Pay Range: $43,600 – $55,930 (commensurate with experience)

At COA, Care Coordinators connect older adults and caregivers to the services that help them live safely and independently. You’ll build trusted relationships, advocate for client needs, and make a lasting impact—while enjoying flexibility, support, and meaningful work.
What You’ll Do
- Conduct assessments to determine program eligibility and care needs
- Develop and manage individualized care plans
- Monitor service delivery and adjust plans as needed
- Coordinate with providers, caregivers, and internal teams
- Advocate for client safety, independence, and well-being
- Maintain accurate documentation and meet program standards
One position will focus on Clinton County; others will serve Hamilton County.

Qualifications
- Bachelor’s degree in Social Work, Nursing, or related field
- OR an Associate’s degree in a human service’s field and at least two years of experience in home health care, medical social work, geriatrics, care/case management or an equivalent substitute
- Experience in care coordination or social services preferred
- Strong communication and organizational skills
- Ability to work independently and collaboratively
- Valid driver’s license and reliable transportation
