Location: Community-based (in-home visits)
Job Type: Full-Time (37.5 hours/week)
Pay Range: $47,000 – $66,000 (commensurate with experience)

What You’ll Do
- Empower clients through personalized care planning and service coordination
- Conduct home visits and phone check-ins to assess well-being and eligibility
- Collaborate with interdisciplinary teams to ensure seamless care transitions
- Educate and support caregivers in navigating health and community resources
- Maintain accurate documentation and uphold program compliance standards
What You Bring
- Education: Bachelor’s degree in Social Work, Psychology, Gerontology, or related human (social) services field
- Experience: At least one year working with individuals with disabilities or chronic conditions
- Preferred: Experience with electronic documentation and case management systems
- Skills: Strong communication, critical thinking, and organizational abilities
- Knowledge: Familiarity with community resources in Southwestern Ohio
- Requirements: Valid driver’s license and reliable transportation to complete community visits across COA’s service area including Hamilton, Butler, Warren, Clinton, and Clermont Counties.
