Job Title: Community Transitions Coordinator – LSW or RN
Location: Community-based (with required client visits and in-person meetings)
Job Type: Full-Time (37.5 hours/week)
Pay Range: $45,000 – $62,000 (commensurate with experience)

At COA, Care Coordinators help older adults and individuals with disabilities return to the community from nursing facilities—empowering them to live with dignity, choice, and support. You’ll work in the community with autonomy, while making a direct impact through client visits and collaboration with healthcare partners.

Job Summary

This role supports the transition of Nursing Facility residents and Home Choice clients back to community settings. You’ll use your clinical expertise and advocacy skills to assess needs, educate families, and coordinate services that promote independence and safety.

Key Responsibilities

  • Conduct person-centered assessments in nursing facilities to explore long-term care options
  • Educate clients, guardians, and caregivers on program eligibility, expectations, and rights
  • Build relationships with hospital discharge planners, PCPs, nursing facilities, and social workers
  • Identify clients ready for transition and coordinate referrals to COA services
  • Provide information and make referrals to community and agency resources
  • Collaborate with caregivers and facilities to support successful transitions home

Qualifications

  • Ohio Licensed Social Worker (LSW) or Registered Nurse (RN) in good standing
  • Minimum of 1 year experience in medical social work and/or geriatrics
  • Strong understanding of healthcare systems and older adult needs
  • Ability to manage caseloads and conduct NF visits and phone assessments
  • Excellent oral and written communication skills
  • Valid driver’s license and reliable transportation for travel within Butler, Clermont, Clinton, Hamilton, and Warren Counties
  • Proficient in Windows OS, MS Word, Excel, and data entry; ability to learn new software
