Job Title: Care Coordination Specialist
Location: Office-based in Blue Ash
Job Type: Full-Time (37.5 hours/week)
Pay Range: $33,100.000 – $42,840.00

What You’ll Do
As a Care Coordination Specialist, you will be the steady, compassionate connection between clients and the services that help them maintain independence. You’ll support care managers, triage incoming calls, arrange essential services, and ensure follow-through—helping clients feel heard, supported, and cared for every step of the way.

What You Bring
- At least one year of experience in social services or direct customer service
- Strong computer and data entry skills (Windows, email, Word, Excel)
- Ability to provide exceptional customer service and communicate with empathy
- Comfort adapting to change in a collaborative, team-based environment
- Valid driver’s license and reliable transportation for occasional travel within Southwestern Ohio
- Respect for diverse cultural, spiritual, racial, and ethnic backgrounds
How You’ll Make a Difference
- Be the first point of contact for clients and caregivers, providing quick, compassionate support
- Arrange and authorize services—including those inside and outside long-term care programs
- Document activities accurately to support quality outcomes and compliance
