Job Title: Assessment & Care Coordinators – LSW, RN, or Human (Social) Services Degree
Location: Community-based in Greater Cincinnati, OH (first 4 weeks in-office training)
Job Type: Full-Time (37.5 hours/week)
Pay Range: $45,000 – $65,000 (commensurate with experience)

What You’ll Do
You’ll help individuals and families navigate care options, coordinate services, and build plans that make life easier and safer—often at pivotal moments. Depending on your experience and interests, roles may include:
- Care Managers
- Assessors
- Waiver Service Coordinators
- Care Coordinators
Work includes a blend of community-based visits, hospital-based assessments, home visits, virtual meetings, and hybrid scheduling depending on the program.
