About the Role

As a PASSPORT Care Manager, you will provide intake, assessment, and ongoing care management services to clients enrolled in the PASSPORT program. You’ll conduct home visits, develop and monitor person-centered care plans, coordinate medical and social services, support caregivers, and ensure individuals receive the services they need to live with dignity and independence.

Responsibilities

  • Provide education, support, and advocacy to individuals with complex social, behavioral, and environmental needs
  • Conduct in-home assessments to evaluate client health status, functional needs, and home environment
  • Use clinical judgment and critical thinking to coordinate physical health, behavioral health, psychosocial, and medical wraparound services
  • Determine ongoing program eligibility and complete comprehensive assessments
  • Develop, implement, and update person-centered care plans with measurable goals and outcomes
  • Monitor service delivery, address provider issues, and ensure services meet clinical and program standards
  • Coordinate transitions between care settings, including hospitals, nursing facilities, and home-based programs
  • Provide coaching and information to help clients and caregivers make informed lifestyle decisions
  • Communicate relevant health and social information to the integrated care team
  • Conduct advanced care planning with clients and involved parties
  • Maintain compliance with confidentiality, HIPAA requirements, and internal policies
  • Complete incident (WIRED) reports timely and accurately
  • Maintain timely and accurate electronic documentation
  • Meet program deadlines for assessments, care plans, and follow-up

Qualifications

  • Ohio Licensed Social Worker (LSW), Registered Nurse (RN), or Licensed Practical Nurse (LPN) in good standing
  • At least one year of experience working with individuals with disabilities, chronic conditions, and/or long-term services and supports
  • Prior case management experience preferred; electronic documentation experience a plus
  • Knowledge of aging, family systems, and issues related to the elderly population
  • Strong clinical judgment, critical thinking, and interviewing skills
  • Above-average technology skills and willingness to learn new software
  • Knowledge of community resources within the region
  • Strong customer service orientation and interpersonal communication skills
  • Ability to adapt to frequent change and thrive in a self-directed team environment
  • Excellent organizational, written, and time management skills
  • Valid driver’s license and reliable transportation for travel within the multi-county area
  • Ability to travel within the state for meetings or training
  • Respect for diverse cultural, spiritual, racial, and ethnic beliefs

Preferred Skills

Pay range $45,000 - $65,000 commensurate with experience
