Position Summary:
Assists veterans and their families in connecting with various services and essential resources in the Greater Cincinnati area to meet their specific needs and improve their circumstances. Serves as a representative of Easterseals Redwood Military & Veteran Services at various community outreach events maximizing our reach and connection in the community, highlighting the organization as the premier provider of veteran services in our area.

Job Responsibilities:
  • Manages all routine requests and administrative follow-up of veteran and/or family member requests for assistance to include performing in-person needs assessments, answering calls, and emails.
  • Determines veteran resource requirements and barriers by completing detailed intake and assessments.
  • Identifies appropriate community resources for veterans and their families and makes referrals based on information provided, personal knowledge, and resource database.
  • Performs client follow-up at appropriate intervals as necessary.
  • Creates, maintains and updates Community OneSource intake/case files for each participant as necessary.
  • Completes required program documentation regarding intake, assessment, referral, resources needed, resources provided, and other follow-up services.
  • Enters documentation into participant's electronic case file, updating milestones, uploading pertinent documents and attachments, creating detailed and informative progress notes, and preparing and submitting monthly reports as required.
  • Assists participants with completing outside agency applications for supportive services as needed.
  • Make decisions about financial assistance requests and responsible for paying approved requests through ESRW MVS. Maintain receipts and detailed record of funds spent.
  • Responsible for scheduling and coordinating Veteran Community Partnership (VCP) meetings including taking minutes and sharing them with team.
  • Time commitment of 30% to supporting HVRP in addition to COS duties, to include writing resumes for veterans, creating/editing enrollment documents as necessary.
  • Solely responsible for large, fast paced, caseload and call volume. Must be highly organized and detail oriented, accurately keep records of call log to ensure no requests are missed and veterans are contacted in a timely manner. Voicemail inbox must be archived daily to ensure space for new incoming voicemails.
  • Maintain a record of Unidentified callers, input into case management system without duplicating. Update Unidentified records with appropriate demographic information as it is obtained.
  • Attends prearranged meetings, resource fairs, and military/veteran social events as Community
  • Ambassador representing Easterseals Redwood Military & Veteran Services when requested.
  • Works varied assigned schedule, exhibits regular and predictable attendance possibly working overtime as required to meet workload demands.
  • Provides administrative support to other Military & Veteran Services internal programs as determined by Manager or Director, Military & Veteran Services
  • Create and maintain a resource list of community partners, military and veteran services, benefits and service organizations.

  • Maintains confidentiality of individual files and other sensitive subject matter.
  • Participates in and coordinate events that highlight Easterseals Redwood Military & Veteran Services as the premier provider of veteran services in our area.
  • Updates job knowledge by participating in educational opportunities; reading publications related to veteran issues and maintaining and growing personal networks.

Education/Qualifications:
  • An appropriate combination of education, training, course work and experience may qualify an applicant to demonstrate required knowledge, skills, and abilities.
  • Bachelor's degree in social work or equivalent of 2 to 3 years of work experience with case management, appropriate resource referrals.
  • A basic understanding of mental health conditions and complex social issues, in addition to the understanding of and, adherence to the NASW code of ethics which guides on how to appropriately work with people addressing socioeconomic issues. Helpful in an understanding of trauma informed care.
