Description
- Screens and completes client intake assessments to include Alcohol and other Drugs (AOD) and Mental Health (MH) assessments, inputs client information into approved database for billing and compliance, administers screening tools as appropriate, develops and monitors case management plans and provide supportive services to individuals on caseload; provides crisis intervention, supportive life skills services; maintain confidentiality and adhere to agreements and standards of the agency; provides resources for essentials, such as food and household supplies; complete and monitor referrals for AOD and/or MH services as needed.
- Performs in-person case management sessions; advocates for clients in court or the community regarding housing, employment or legal issues.
- Assist in data collection for necessary reports and plan program meetings to review client progress. Ensures records and documentation is complete and maintains client information in HMIS systems, including vital statistics, and services requested and received for other reports. Conducts 3-month assessments and ongoing review of case plans.
- Make weekly or bi-weekly contact with clients (office or community); monitor case management plans to ensure goal achievement.
- Collaborate with partner agencies, organizations and case managers, Social Security, Mental Health Board, and other appropriate organizations and individuals as needed.
Qualifications
- Peer Support Specialist certification and experience, life skills certification or Qualified Mental Health Specialist (QMHS) certification are highly preferred but not required.
- Bachelor’s degree in criminal justice, counseling, human services, psychology or social work and two years case management experience required; associate’s degree in human services or related field with at least three years of direct case management experience; LSW strongly preferred for MH billing and compliance. Experience in providing case management services to MH/SUD populations. Ability to manage multiple tasks and work effectively with a diverse population. Experience working with transitional housing providers is helpful. Proficient in all Microsoft software (Outlook, Word and Excel).
- Excellent oral and written communication and interpersonal skills. Ability to provide outreach casework services, including counseling and emergency housing. Able to perform assessments, advocacy, housing counseling, and referrals. Able to develop and maintain accurate casework files and develop reports. Basic math and budgeting skills. Experience in developing community-housing resources.
