General Summary

The Mercy Health Partnership Program (MHPP) provides patient advocacy and education, co-creates action plans with patients, and lowers barriers to care and optimal health. The Community Health Worker will assist referred patients with access to care including insurance acquisition and optimization, financial assistance, and connection to community resources. The Community Health Worker will be an integral part in assisting patients with health-related social needs as identified by screenings at Mercy Health clinical care sites.

Employment Qualifications:

  • Certified Community Health Worker - if not certified required certification within 12 months of hire, which will be supported through the program.
  • Minimum years and type of experience: 3 years - preferred
  • Valid Ohio driver's license and one year of driving experience and no traffic citations.
  • Valid automobile insurance.
  • Sensitivity and experience in working within different cultures. Ability to communicate orally and in writing in English. Proficient with computers and accuracy with data entry and Microsoft Office. Ability to work independently or with little supervision.
  • Ability to separate personal from professional interactions with clients and maintain professional/ethical boundaries. Ability to document client interactions with accuracy and in a timely manner. Ability to learn and implement new procedures and adapt to emerging community needs.

Essential Job Functions

  • Patient Engagement (40%): Complete telephonic and in-person encounters to address patients’ social needs, including obtaining and optimizing insurance coverage, connecting to community resources, and providing patient education and advocacy.
  • Community-Clinical Connection (20%): Establish trusting relationships with patients and their families and maintain excellent working relationships with patients and staff. Serve as a patient connection between the CHWs and Social Workers within the program, the patient’s care team, and community-based organizations to ensure patients’ needs/barriers are being addressed.
  • Reporting (5%): Work with the MHPP team to capture data required and provide to Manager in a timely manner each month.
  • Documentation (20%): Document in the patients’ electronic medical record and other care coordination systems as required for comprehensive, quality service and payment.
  • Trainings (15%): Attend meetings and trainings as required by MHPP, Community Health Department, and CHW Hub. Foster good working relationships with the community and other service providers.
