Social Worker LISW - The Family Nurturing Center - Mercy Health Physicians

SUMMARY OF JOB RESPONSIBILITIES:

Under the direct supervision of the administrative director (or his physician designee) and administrative direction of the Department Director, provides outpatient clinical services (individual, group, family, couples) to patients at a Mercy Health Physicians Outpatient Clinic Site in Lorain, Ohio. Completes all medical records documentation required for diagnostic reassessment, treatment planning, referrals, transfers, discharge/termination planning, and quality/performance improvement. Fulfills all requirements for advanced pre-certification and continuing treatment authorizations with third-party payers.

QUALIFICATIONS:

EDUCATION:

  • Masters Degree from an accredited training program in area of speciality.
  • LISW or LPCC by Ohio Counselor and Social Work Board and 2-4 years post licensure experience performing same or related job duties (i.e. outpatient counseling).

OR

· CNS with Certificate of Authority in psychiatric nursing/mental health practice.

EXPERIENCE:

  • Managed Care and HMO/Capitated Treatment Planning.
  • Use of brief therapy models.

· Ability to work with a wide range of patient age groups and functional levels

KNOWLEDGE AND SKILLS:

Has in-depth knowledge of recognized professional field.

Applies advanced knowledge in practice.

Understands and integrates critical information from multiple disciplines.

Applies advance knowledge and skills to achieve cost effective, positive patient outcomes.

· Problem Solving/Decision Making:

Exercises significant risk-taking, judgement, and creativity to identify and seek problem resolution.

Makes decisions on complex, sensitive issues.

Collaborates with Program Director and Department Chairman on identifying both short and long-term objectives.

Develops solutions and/or courses of action.

· Scope of Impact/Accountability:

Serves as key advisor on practice patterns and issues/concerns.

Impacts practice in Clinic.

Accountable for own practice.

PRINCIPAL JOB ACCOUNTABILITIES:

  • Spends at least 80% of worked time to deliver direct, billable treatment services to Mercy Health Physicians, in compliance with Department and BSMH policies, procedures, and Standards of Practice.

  • Completes all medical records documentation (content and timeframe requirements) mandated by Departmental BSMH regulatory (e.g. licensure, certifications, accreditations, legal), and third-party payer (managed care, HMO, Medicare, commercial insurance, etc.) agents.

  • Provides at least two psychotherapy groups, reviewed, and approved in advance by BSMH. Performance measured by written description group (narrative, schedule, admission criteria, therapists, etc.) Scheduling system and charge reports.

  • Pre-certifies admission of outpatients for treatment in advance of visits and obtains authorizations for continuing care, from third party payers according to contractual obligations. Performance measured by documented pre-certification/continuing certification number or letter, specifying length and frequency of authorization.

  • Provides high-quality services to Clinic Outpatients, in compliance with Departmental, BSMH, licensure, and accreditation agents and with accepted standards of practice. Performance measured by patient satisfaction, medical records, documentation, and audit/survey results.

  • Provides scheduled work hours during clinic hours to accommodate patient needs and service demands. Performance measured by master schedule review.

  • Completes all BSMH and Department treatment and provider-related work (e.g. provider applications, reports to third-parties, legal documents) within timeframes requested for response. Performance measured by stamped date in requestors office when information is received.

  • All other duties as assigned.
