About the opportunity
Mercy Health is seeking experienced and passionate Clinical Care Coordinator to join our dedicated team at Mercy Place Dandenong Aged Care Facility in Melbourne.
As a Clinical Care Coordinator, you will lead and manage the clinical care operations of our facility under the supervision of Clinical Manager . Your role is pivotal in ensuring the highest standard of person-centred care is delivered, and you’ll work closely with residents, families, and a multidisciplinary team to ensure all aspects of physical, emotional, and social wellbeing are addressed.

To thrive in this role, you will have:
Previous experience as CCC in aged care setting.
Proficiency in computer applications such as Microsoft Office (Word, Excel, Outlook).
Oversee the day-to-day clinical operations, ensuring compliance with all relevant legislation, policies, and procedures.
Develop, implement, and regularly review individualised care plans in collaboration with healthcare professionals and family members.
Lead, support, and mentor a team of dedicated care staff, fostering a culture of continuous improvement.
High level of interpersonal, organisational & communication skills (written and Verbal )
High attention to detail and previous knowledge of care planning.
Excellent time management skills and an ability to manage conflicting priorities.
A sense of empathy & compassion for elderly residents & their families.
Collaborate effectively with multidisciplinary teams to coordinate and optimise care outcomes.
You will also be required to provide evidence of, or in the process of obtaining:
National Disability Insurance Scheme (NDIS) Clearance.
Current registration as a Registered Nurse with AHPRA.
Minimum 3 years’ experience in a clinical leadership or management role.
Strong analytical and problem-solving capabilities.
In-depth understanding of aged care regulations, accreditation standards, and best practices.
Proof of current influenza vaccination and other vaccination status consistent with public health orders.
