Position Summary:
Provides and manages all aspects of patient care in accordance with State and Federal rules and regulations and the nursing standards based on the scope of a Registered Nurse, under the direction / delegation of the NP or MD.
Job Responsibilities:
- Assesses, evaluates and identifies patient care requirements by establishing personal rapport with potential and current patients; triages medical needs in an accurate and precise way through patient history, interviewing and careful examination; assist with components of physical exam as ordered by NP or MD
- Assures quality of care by adhering to therapeutic standards; measuring health outcomes against patient care goals and standards; making and recommending necessary adjustments; following agency and medical department philosophies and standards of care set by state board of nursing, state practice act, and other governing agency; monitor vital signs (temperature, blood pressure, pules and respiration) as ordered by NP or MD
- Administer medication accurately, observe, monitor and evaluate responses including side effects, involuntary movements and adherence to prescribed medication; log medications, audit, document and coordinate prescriptions with external pharmacies,
- Collaborate in sharing information with other members of the medical team; assist in development of solutions to ensure health, comfort and safety of patients served; coordination of patient referrals; assist in facilitating the patients’ transition within and between health care settings (admitting, transferring and discharging patients)
- Promote patient independence by establishing patient goals and teaching patients, friends and family to understand the condition, medications and self-care skills; act as a patients’ advocate for families, individuals, groups and communities; educate patients about preventive care and prescribed treatments; Interpret and educate treatment plans, test/ lab results, medications and diagnoses of patients’ individualized treatment plan
- Protects patients and employees by adhering to infection-control policies and protocols, medication administration and storage procedures, and controlled substance regulations; oversee management of adequate inventory of medical supplies and medications
- Documents patient care services by charting in patient and department records; manage and maintain patient health records to ensure compliance
- Provide oversight, delegation and direction to LPN and MA; ensure use of established procedures, policies, guidelines and standards under the disciplines scope of practice
- Other duties as assigned
Required Knowledge, Skills and Abilities:
- Excellent written and verbal communication skills

Job Requirements:
- Associate’s Degree in Nursing, Bachelor’s preferred
- Minimum 2 years’ nursing experience
- Active Registered Nurse license in Ohio
- Valid driver’s license and current auto insurance
Essential Functions/Physical Demands:
- Positional: Driving in accordance with job duties assigned. Frequent standing and walking. Infrequent sitting.
- Gross Mobility: Rare lifting, climbing, or crawling. Infrequent balancing, stooping, kneeling, or crouching. Frequent to constant reaching, bending and handling.
- Sensory: Rare tasting/smelling. Infrequent use of color vision. Occasional use of far visual acuity, depth perception, and field of vision. Frequent use of near and midrange visual acuity, and visual accommodation. Frequent to constant talking.
