| Position Summary: Foster and maintain ongoing relationships with family members, service providers, hospitals, courts, education system providers, and other identified resources to facilitate and implement the Wraparound process. |
| Position Description: - Cultivate flexible, family-focused, community-based responsive services
- Develop and maintain the Wraparound Team, including coordinating and leading team meetings
- Coordinate and supervise implementation of the Plan of Care, including a Transition Plan and Crisis Plan, through service delivery with providers and community resources; update plan as necessary
- Ensure family support and stabilization during crises
- Provide and document the initial and ongoing Life Domain Assessment
- Maintain all service documentation requirements, evaluation outcome requirements and data as required
- Provide services in a timely manner and in accordance with Plan of Care and/or Crisis Plan
- Utilize and monitor Flexible Funding and service coordination
- Obtain weekly reports from subcontracted providers
- Participate in after hours on-call response
- Attend Program staff meetings, supervision and any other meetings as required
- Participate in the Agency and Program CQI Peer review process
- Perform duties to reflect Agency policies and procedures and comply with regulatory standards
- Meet Agency training requirements
- Report all MUIs to Site Manager and Supervisor immediately
- Other duties as assigned

| | Job Requirements: - Bachelors degree in related field and two years of direct experience or five years of related experience
- Must complete High Fidelity Wraparound model training within 12 months of employment.
| | Essential Functions/Physical Demands: - Positional: Driving in accordance with job duties assigned. Frequent walking and use of stairs. Frequent to constant sitting.
- Gross Mobility: Rare climbing, or crawling. Infrequent balancing, stooping, kneeling, or crouching. Occasional to frequent reaching, handling and fingering.
- Sensory: Rare feeling, tasting/smelling and use of color vision. Occasional use of midrange and far visual acuity, depth perception, and field of vision. Constant talking and hearing.
